Band 6 Children's Community Nurse

Overview

This is a permanent role with full-time and part-time opportunities (total 1.4 WTE) for an experienced Children’s Nurse to join the Children’s Community Nursing Team (CCNT) in Redbridge. The post holder will work autonomously in the home setting, providing family‑centred, high‑quality nursing care to children and young people with complex needs. The role is supported by a Band 8a Lead Nurse and a senior multidisciplinary team.

• Deliver direct clinical care to children and young people in community settings, including both acute and long‑term conditions.
• Manage a defined caseload of patients with complex health needs and disabilities, ensuring continuity of care.
• Implement effective case management to reduce avoidable hospital admissions.
• Collaborate with families, carers and a wide range of multidisciplinary and multi‑agency professionals to achieve best outcomes.
• Maintain patient records in a new electronic system and support clinical governance.
• Participate in ongoing professional development and contribute to service improvement.

• Full, clean UK driving licence and access to a vehicle.
• Proven experience working as a Children’s Nurse, with a commitment to delivering high‑quality, family‑centred care.
• Excellent communication skills and a positive approach to service development.
