Erewash Health Partnership are looking to recruit enthusiastic and motivated individuals to join our team as a Long Term Conditions Care Coordinator, supporting either our North or South PCN Hub, depending on the preference of the applicant, with part and full-time roles available.
The postholder will work to support PCN-wide services to patients registered in the following practices:
- Little wick Medical Centre
- Old Station Surgery
- West Hallam Medical Centre
- South Street Surgery
- Gladstone House Surgery
- West Park Surgery
- College Street Surgery
- Adam House Medical Centre
- Aitune Medical Practice.
The post-holder must be committed to providing an excellent service to our patients and possess excellent team working and communication skills.

About usErewash Health Partnership (EHP) was established on 1 April 2018 and is a single GP Partnership comprising of seven constituent practices. EHP employs approximately 240 people who work within the constituent practices and central team, supporting services delivered across Erewash Primary Care Network, a PCN of 11 practices serving over 100,000 patients.
Location:
Primarily Little wick Medical Centre (North) or Sawley Medical Centre (South), although the post holder may be required to work at any establishment at any time throughout the duration of their employment.

Person Specification Experience- Experience of working in a public facing role.
- Experience of working within multi-professional team environments.
- Experience of working in primary care.
- Experience of working in a GP practice.
- Experience of working in health, social care, and other support roles in direct contact with people, families or carers (in a paid or voluntary capacity) is desirable.
- Experience of supporting people, their families and carers in a related role.
- Good basic education.
- Grade A-C in Mathematics and English or equivalent.
- NVQ Level 3 in adult care - advanced level or equivalent qualifications or working towards is desirable.
