Certified Diabetes Educator/Dietitian, Full-Time

Responsibilities
  • Provide diabetes self‑management education to patients with Type 1, Type 2, and Gestational Diabetes.
  • Offer non‑diabetes related Medical Nutrition Therapy services to patients aged 2 years and older.
  • Conduct proficient assessments of health history, resource use, risk factors, cultural influences, health beliefs and behaviors, support systems, socioeconomic factors, dietary habits, and learning barriers.
  • Educate patients in individual and group settings on basic diabetes self‑management concepts, including healthy eating, physical activity, medication administration (including injectable medications and insulin), monitoring (glucometer and CGM instruction), problem solving, healthy coping, and reducing risk of complications.
  • Document the essential assessment, instructional content, response to interventions, and the plan for follow‑up in the electronic medical record.
  • Utilize patient data, current research, and goal‑setting to meet the educational needs of diabetes patients and their significant others.
  • Assess, interact with, and care for patients in a manner that demonstrates an understanding of the patient’s readiness for behavior change, using current educational and goal‑setting techniques to assist with behavior change and self‑management.
  • Collaborate with interdisciplinary team members in the plan of care for diabetes patients.
  • Teach/facilitate diabetes group classes and assist in other health education opportunities within Duly Health and Care and the surrounding community.
  • Demonstrate practices consistent with the professional associations (ADA, ADCES) Standards of Clinical Care.
  • Establish relationships with representatives in diabetes care and nutrition to maximize knowledge of new strategies for diabetes and/or nutrition management.
  • Complete the mandatory continuing education hours established by the Association of Diabetes Care and Education Specialists and the Commission on Dietetic Registration to maintain diabetes care and education specialist certification and dietitian registration.
Qualifications
  • Proficient PC and keyboard skills and familiarity with Microsoft Office applications.
  • Excellent communication and writing skills.
  • Ability to plan, organize, and work independently.
  • Strong problem‑solving skills.
  • Maintains current knowledge of professional association recommendations and insurance billing requirements for professional services.
  • Ability to develop professional rapport with employees at all levels of the company.
  • Flexible to work some evenings and occasional weekends.
Education and Certification / License
  • Dietitian with a bachelor’s degree from a four‑year college or university or an equivalent combination of education and experience.
  • Current certification through the Certification Board for Diabetes Care & Education (CDCES) or actively working toward certification.
  • Current license from the Illinois Department of Education and Registration.
  • Current registration through the Commission on Dietetic Registration.
  • Current CPR certification.
Experience

Prefer at least 2,000 hours of direct diabetes patient education. Knowledge of insulin pumps and experience with continuous glucose monitoring devices. Familiarity with American Diabetes Association Program Recognition Standards.

Compensation

The anticipated base compensation range is $32–$55 per hour, depending on qualifications and experience. The position may be eligible for a sign‑on bonus.
