Licensed Social Worker – $7,500 Sign‑On Bonus

Competitive Pay: $29.00–$35.00 per hour. Bonuses or other incentives are offered separately. All employment offers consider experience, qualifications, location, certifications, and training.

• Coordinate medically related social services to help residents manage physical, mental, and psychosocial needs.
• Assist in meeting the psychosocial needs of residents, families, and others during illness, disability, and treatment.
• Address advance directives and help determine appropriate medical decision makers.
• Provide individual, family, and group services to maximize psychosocial well‑being.
• Promote staff actions that protect resident rights and dignity.
• Help develop interdisciplinary care plans for residents.
• Offer support and grief counseling as indicated.
• Oversee and coordinate discharge planning to all other settings.

• Hold a bachelor’s or master’s degree in Social Work.
• Preferred experience in a skilled nursing or sub‑acute health‑care setting.
• Current valid Maryland Social Work license.
• Ability to function as a member of an interdisciplinary team.
• Excellent interpersonal skills with residents, families, and community agencies.
• Strong prioritization, organization, and ability to handle distractions.
• Proficiency in using electronic health record systems.
