The Team Manager is the leader of the Patient Care Team and responsible for supervising, evaluating, and coordinating the interdisciplinary team, ensuring continuity of care, advocating for patients, and maintaining records.

• Direct all patient care services and ensure integrated care.
• Review admissions and discharges for eligibility.
• Collaborate with physician for palliative care.
• Ensure accurate data for recertification decisions.
• Plan discharges and monitor services.
• Manage team schedule for 24/7 service.
• Facilitate team meetings for care plan updates.
• Participate in on-call rotation.

• Interview, select, train, supervise, evaluate, and dismiss staff.
• Maintain staff competence through field visits and documentation.
• Provide mentorship on clinical issues.
• Oversee staff schedules and workload distribution.
• Approve payroll and manage time records.

• Monitor records and compliance with policies.
• Implement performance improvement activities.
• Conduct patient/family satisfaction surveys and apply quality measures.
• Assist with regulatory surveys and chart reviews.
• Participate in program reviews and outcomes management.
• Perform utilization reviews for continuous and inpatient care.

• Address grievances and ensure prompt resolution.
• Analyze customer service issues and improve performance.
• Engage patients/families when considering revocation.
• Visit LTC and contract facilities to discuss care plans.
• Provide customer service and service organization participation.

• Verify admissions diagnosis and treatments.
• Monitor utilization of resources.
• Control labor and patient care costs.
• Approve invoices relating to patient care services.

• Attend educational seminars and workshops.
• Set professional growth goals.
• Mentor newly hired Team Managers.

• Reliable transportation with driver license and insurance coverage.
• Private telephone in home.
• Knowledge of primary medical and nursing care, oncology nursing, and palliative care.
• Understanding of interdisciplinary dynamics, grieving process.
• Working knowledge of VITAS information system (Vx) and reports.
• Two years supervisory experience in a health care organization.

• Bachelor's degree preferred.
• Current valid RN license to practice in the state.
