We are seeking an experienced and compassionate Registered Nurse (Home Health) to join our client’s team in Carlinville, Illinois. This position focuses on delivering personalized nursing care to patients in their homes, helping them achieve optimal health outcomes while maintaining comfort and independence. The ideal candidate will be self-motivated, detail-oriented, and committed to providing exceptional, patient-centered care.
Key Responsibilities- Provide skilled nursing care in patients’ homes based on individualized care plans.
- Conduct comprehensive patient assessments and evaluate ongoing needs and progress.
- Educate patients and caregivers on disease management, medications, and home safety practices.
- Maintain accurate and timely clinical documentation in compliance with healthcare regulations.
- Communicate effectively with interdisciplinary team members to ensure coordinated care.
- Participate in team meetings, case conferences, and training opportunities to support continuous professional development.
- Monday–Friday, daytime hours.
- Includes one weekend per month, 2–3 on-call days per month, and two holidays per year.
- This is a Pay Per Visit position.
- Local travel required within the Carlinville area.
- Associate degree or higher in Nursing.
- Active Registered Nurse (RN) license in Illinois.
- Minimum of 1 year of professional RN experience (home health preferred).
- Current Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification through the American Heart Association.
- Valid driver’s license, reliable transportation, and auto insurance.
- Previous experience in home health or community-based nursing.
- Strong clinical assessment, communication, and time management skills.
