We are seeking a compassionate and skilled healthcare professional to join our team in a vascular care setting. In this role, you will provide high-quality, patient-centered care for individuals with vascular conditions, collaborating closely with physicians, nurses, and other members of the care team to support accurate assessment, treatment planning, and ongoing management. Vein and Vascular Centers have locations in Hinsdale, Downers Grove and Plainfield.
Looking for a provider part time or full time.
- Perform comprehensive patient assessments, including history taking, physical examinations, and interpretation of diagnostic data related to vascular health.
- Develop, implement, and adjust care plans in collaboration with the supervising or collaborating physician and the care team.
- Evaluate and manage patients with acute and chronic vascular conditions in outpatient, inpatient, or procedural settings as applicable.
- Order, interpret, and follow up on diagnostic tests, imaging studies, and laboratory results.
- Provide education to patients and families regarding vascular disease prevention, treatment options, medications, lifestyle changes, and post-procedure care.
- Document assessments, plans of care, treatments, and patient outcomes accurately and in a timely manner.
- Monitor patient progress and communicate changes in condition to the appropriate members of the healthcare team.
- Support pre- and post-operative care, including wound assessment, symptom monitoring, and coordination of follow-up care.
- Assist with quality improvement initiatives and evidence-based practice efforts to enhance patient outcomes.
- Maintain compliance with clinical standards, organizational policies, and applicable regulatory requirements.
- Master's degree or higher from an accredited nurse practitioner program.
- Current licensure as a registered nurse and advanced practice registered nurse, or equivalent licensure as required by the state.
- National certification as a nurse practitioner in an appropriate population focus.
- Clinical experience in vascular care.
- Strong assessment, diagnostic reasoning, and care coordination skills.
- Ability to work effectively in a multidisciplinary team environment.
- Excellent communication skills and a commitment to patient education and compassionate care.
- Proficiency with electronic health records and clinical documentation practices.
- Ability to prioritize, manage multiple responsibilities, and adapt in a dynamic clinical setting.
- Experience supporting complex vascular patients across the continuum of care.
- Knowledge of vascular procedures, wound management, and post-operative recovery protocols.
- Familiarity with evidence-based guidelines and quality improvement practices.
- Strong organizational skills and attention to detail.
