• Conduct comprehensive patient assessments to develop personalized care plans.
• Provide therapeutic treatments and interventions in the patient's home environment.
• Collaborate with patients, families, and healthcare teams to achieve optimal outcomes.
• Document patient progress and modify treatment plans as necessary.
• Educate patients and their families on exercises and strategies to enhance mobility and function.
• Ensure compliance with all regulatory and professional standards of care.

• Valid state license as a Physical Therapist.
• Proven experience in a home health setting is preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and manage a flexible schedule.
• Excellent organizational skills and attention to detail.
• CPR certification is required.
• Valid driver’s license and reliable transportation.
