Overview

Join the Centerpoint Medical Center team as a Clinical Pharmacist Float. This role supports inpatient medication therapy and collaborates with clinical staff to improve patient outcomes. • You will interpret orders and transcribe into the pharmacy computer system.
• You will accurately compound and dispense pharmaceuticals.
• You will monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.
• You will suggest appropriate, cost-effective therapeutic medication alternatives to medical staff.
• You will sustain the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and promote rational drug therapy selection.
• You will assist with clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as defined by Pharmacy Leadership.
• You will complete notification reports regarding medication errors and adverse drug reactions.
• You will provide oversight of staff schedules.

• B.S. or Pharm
  
  D from an ACPE-accredited School of Pharmacy, a Doctor of Pharmacy from an ACPE-accredited college of pharmacy is preferred.
• Active pharmacist license in good standing with the appropriate State Board of Pharmacy.
• State Specific Medications Therapy Management Certification.
• At least one of the following:
  Minimum of 1 year of hospital clinical pharmacy experience or at least 2 years of general hospital pharmacy experience preferred; within area of focus.
• Completion of hospital PGY1/ pharmacy general practice residency or fellowship highly preferred.
• Specialty BCPS certification preferred.
