• Monitor residents’ mental and psycho‑social needs and provide services to maintain the highest level of physical, mental, and psycho‑social well‑being.
• Apply knowledge of age‑specific developmental factors for adult and geriatric residents to direct the overall operation of the facility’s delivery of care.
• Participate in pre‑admission screening of all potential residents, provide information to residents and families during admissions, orient residents to the long‑term care environment and facilitate adjustment.
• Coordinate and review behavior charting to ensure an effective plan of care is implemented.
• Provide therapeutic counseling to residents and family members as appropriate to meet residents’ psych‑social needs.

• Degree in Social Services or a related field (or equivalent professional experience).
• Capability to perform the essential functions of the job.
• Proficiency in spoken and written English.
• Strong knowledge of state, federal, and local regulations pertaining to long‑term care.
• Good communication skills, patience, and self‑discipline.
