Social Services Assistant
Outfield Healthcare Partners is seeking a Social Services Assistant to join our skilled nursing team in Independence, Missouri. This role supports the Social Service Director and Social Workers in providing medically related social services to residents, helping them achieve the highest level of psychosocial well-being, dignity, and quality of life.
This is an excellent opportunity for someone with experience in long-term care or skilled nursing who is passionate about resident advocacy and compliance-driven social services.
Key Responsibilities- Assist the Social Service Director/Coordinator with resident assessments, care planning, and documentation
- Maintain accurate, timely records including MDS, care plans, social histories, progress notes, and behavior tracking
- Support resident rights, self-determination, and informed decision-making
- Assist with admissions, discharge planning, and transitions of care
- Coordinate referrals to community resources, home health, hospice, and social service agencies
- Work closely with residents and families to provide education, support, and advocacy
- Participate in interdisciplinary team meetings and quality assurance activities
- Promote resident dignity, cultural sensitivity, and abuse prevention
- Report suspected abuse, neglect, or exploitation per state and federal guidelines
- Support survey readiness and assist during state and federal inspections
- Collaborate with nursing leadership and administration to improve resident quality of life
- Educate residents and families on advance directives and resident rights
- Ensure confidentiality and HIPAA compliance
- Respond promptly and professionally to resident concerns and grievances
- Maintain up-to-date community resource listings
- Support psychosocial interventions during significant life events or crises
- Social Service Certificate or 36-Hour Social Service Designee Certificate (preferred)
- 1+ year of supervised social services experience in a healthcare or long-term care setting
- 2+ years of Skilled Nursing Facility (SNF) experience preferred
- Knowledge of federal and state regulations governing social services in long-term care
- Strong documentation, communication, and organizational skills
- Ability to work collaboratively with interdisciplinary teams and families
