Make a Difference Where It Matters Most
We’re looking for a Nurse Practitioner (NP) or Physician Assistant (PA) to provide compassionate, high-quality medical care to residents in skilled nursing facilities. This role supports a variety of residents, including those receiving post-acute care, rehabilitation, long-term care, and care for complex medical conditions.
You’ll work closely with facility nursing teams, physicians, Medical Directors, and other members of the interdisciplinary care team to evaluate resident needs, manage acute and chronic conditions, and provide ongoing clinical support.
What You’ll Do- Review and organize your daily schedule across assigned facilities, prioritizing resident visits based on clinical urgency, continuity of care, and resident needs.
- Collaborate with Nursing Directors, Medical Directors, and facility teams to identify changes in condition, acute needs, and additional medically necessary visits.
- Evaluate, diagnose, treat, and follow up with residents within your scope of practice.
- Prescribe and manage medications, approve refills, and order or approve laboratory testing, diagnostic studies, treatments, therapies, and referrals as clinically appropriate and permitted by law.
- Communicate significant findings, treatment recommendations, changes in condition, and follow-up needs with facility staff, attending physicians, Medical Directors, residents, and families.
- Complete timely and accurate electronic documentation, sign charts, and submit charges within required time frames.
- Participate in a shared on-call rotation with the Medical Director and other designated providers.
- Maintain accurate on-call documentation and complete required administrative reporting.
- Travel between assigned skilled nursing facilities as needed.
- Graduate of an accredited Nurse Practitioner or Physician Assistant program.
- Current, unrestricted NP or PA license in each state of practice.
- Current national board certification appropriate to your discipline.
- Current DEA registration and controlled-substance authority, when required.
- Current BLS certification.
- Adult or geriatric clinical experience; skilled nursing, long-term care, or post-acute care experience is preferred.
- Strong clinical judgment and the ability to work independently while knowing when to collaborate or elevate concerns.
- Excellent communication, organization, and time-management skills.
- Proficiency with electronic medical records and clinical documentation.
- Reliable transportation for travel between assigned facilities.
This is an opportunity to build meaningful relationships with residents and care teams while providing hands-on clinical care in a supportive skilled nursing environment. If you’re looking for a role where your clinical expertise can directly improve the lives of older adults and medically complex patients, we’d love to hear from you.
