Work weekends (Friday, Saturday, Sunday) on 12‑hour shifts: 10 AM – 10:30 PM.
ResponsibilitiesThe Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside (private homes, Skilled Nursing Facilities, and assisted living facilities). This position is a contributing member of the interdisciplinary team, providing expertise regarding psych‑social assessments, interventions, and knowledge of community resources.
Qualifications- Current Registered Nurse (RN) licensure in good standing in the state(s) of practice
- Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
- Works closely with patients on a 1‑to‑1 level while receiving strong support from the patient care team
- Can work independently and autonomously with activities such as phone, email, written communication, meetings, and understanding of hospice rules and regulations
- Time‑management skills to coordinate visits, documentation, meetings, and prioritize urgent needs by triaging multiple concerns
- Ability to learn an electronic medical records system
- Willingness to drive with reliable transportation, valid driver’s license, and auto insurance for travel across the service area
