Overview

Continuum Rehab Group provides in‑house therapy with Skilled Nursing facilities across Texas, New Mexico, Indiana, Arizona, and Missouri.

Base pay range: $30.00/hr - $36.00/hr

• Utilizes evidence‑based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment interventions to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of a Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and documents patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.

Licensed Physical Therapy Assistant. Position open to therapists at all experience levels.
