LPN - Care Coordinator at Neighborhood Health Center

General Summary An “LPN Care Coordinator” coordinates the patient transition along the healthcare continuum through assessments, referrals and interdisciplinary planning for patients with chronic diseases. The LPN Care Coordinator is responsible for development, implementation & revision of the patient care plan in collaboration with the interdisciplinary team.

Pay From $28.01 per hour

Status Full-time, non-exempt, Monday‑Friday

Essential Responsibilities
  • Assesses the care transition needs of the patient
  • Develop and implementation of the plan of care
  • Revision of the care of plan as needed
  • Develop patient centered goals with the patient and support system
  • Collaborates with Interdisciplinary Team in development of the transition plan of care
  • Coordinates post-hospitalization services
  • Accompanies patients to physician visits as needed
  • Fosters an environment of individual responsibility
  • Re-enforces with patients the physician prescribed treatment and discharge plan
  • Completes routine reviews per acuity

    Ensure documentation is complete to meet billing requirements
  • Provide community resources as needed
Secondary Function

Supports the Clinical staff under the direction of qualified medical personnel within the scope of skills and abilities.

Other duties as assigned.
