We’re hiring a Nurse Practitioner (NP) or Physician Assistant (PA) to support our dementia care program
. This hybrid role combines clinic-based and telehealth visits, with opportunities to expand into senior living communities. You’ll conduct cognitive evaluations, manage behavioral and chronic conditions, and educate families navigating dementia.

• Conduct 45-minute cognitive assessments (CPT 99483)
• Deliver dementia-focused care via clinic and telehealth
• Coordinate with PCPs and care partners
• Manage follow-up visits and documentation
• Support senior living expansion initiatives

• Base salary, dependent on experience and qualifications
• Schedule:
  
  Monday–Friday, 8 AM–5 PM | No call or weekends
