Fox Rehabilitation is seeking a licensed Physical Therapist to provide one-on-one, in-home and senior living care in Goshen, NY, delivering evidence-based therapy and coordinating with an interdisciplinary team.
You will design care plans, evaluate progress, and document in FOX’s EMR while promoting aging in place for older adults. Telehealth may be incorporated to extend access and flexibility.
