Role Overview
Pay Rate: $22-$25/hour, depending on language proficiency (Spanish fluency at $25/hr).
Work Type:
Full-time, in-person, community-based,
Schedule:
Monday–Friday, 9AM–5PM.
Role
Summary:
The Integrated Health Coach supports individuals with behavioral health challenges by building trusting relationships, understanding personal goals, and connecting members to resources to promote recovery and self-determination. Coaches engage members in their communities and via phone, providing exceptional advocacy, support, and encouragement while collaborating with clinicians and specialists to ensure comprehensive care.
- Engage and build trust with members in the community.
- Achieve enrollment and engagement targets to support program growth.
- Assist members in creating and tracking personal recovery plans.
- Model positive behaviors and provide ongoing encouragement.
- Offer wellness strategies and facilitate physical and behavioral health management.
- Provide physical health coaching following training, if needed.
- Offer feedback to improve processes and support continuous enhancement.
- Over two years of experience working with individuals diagnosed with serious mental illness.
- Experience in community settings within Marion County.
- Valid driver’s license and high school diploma or equivalent.
- Nice to have:
Spanish fluency, Certified Peer Specialist training, experience with Substance Use Disorders.
