Summit Home Care is seeking a dedicated Physical Therapist (PT) to join our home-based care team. The position is PRN with on-the-road patient visits and training at the local office or remotely. Travel is required with mileage reimbursement.
Pay is per visit, and the location includes Tipton, IN and Howard, IN. Responsibilities include evaluating patients, developing rehab plans, assessing home safety, teaching patients and caregivers, and coordinating with physicians.
