As an Assessment Counselor here, you are the gateway to our recovery community. You will:
- Coordinate Clinical Care: Work closely with interdisciplinary teams and external providers to ensure seamless updates on clinical information.
- Drive Treatment Planning
:
Lead the admission process by completing thorough psychosocial assessments and helping build a roadmap for recovery. - Connect the Community
:
Provide vital resource information and "warm-referrals" to community providers to ensure ongoing engagement. - Manage Admissions
:
Handle the intake process with excellence, including verification of benefits and necessary clinical authorizations.
- Bachelor's degree (LSW or RN preferred), or Master's degree in social work or counseling with licensure.
