Position: OUTREACH SERVICES COORDINATOR
Location: Indianapolis

Position Objective

The Outreach Services Coordinator position is accountable to the client, Horizon House, and the Professional Blended Street Outreach (PBSO) Team. Accountability occurs within the context of ethical standards, organizational policies and procedures related to the delivery of service, PBSO standards of practice, and the written agreement between Horizon House and relevant funder(s).

This Outreach Services Coordinator position is 100% funded through by a grant from the Nina Mason Pulliam Charitable Trust (The Trust). As such, this position is dependent on execution and continuation of said grant agreement, and termination or nonrenewal of the agreement will result in termination of the Outreach Services Coordinator position.

Requirements Training/Education
  • Bachelor’s degree preferred; commensurate experience will be considered
  • Case management experience (3-5 years)
  • Homeless services experience strongly preferred
  • Bi-lingual (English/Spanish) a plus
Experience/Skills
  • Valid Indiana driver's license
  • Reliable transportation and valid auto insurance, per agency guidelines
  • Ability to work a flexible schedule including some evening and weekend hours
  • Working knowledge of chronic medical and mental health conditions, substance use disorder and best practice approaches for addressing these barriers to housing stability
  • Excellent verbal and written communication skills and the ability to communicate with diverse audiences including clients and community partners and the ability to coach team members in the same
  • Strong commitment to best practice models, ethical decision-making and measurable outcomes
  • Strong organizational skills
  • Ability to assess for and address potential hazards/crises using effective problem-solving and de-escalation skills, and coach team members to do the same
  • Skill working in a team environment, managing group dynamics and inspiring teamwork in others
  • Proficiency in all Microsoft applications and the ability to quickly become proficient in electronic case management system
  • Ability to demonstrate cultural sensitivity and respect clients  self-determination
Special Requirements
  • MUST HAVE a valid Indiana operator’s license in good standing and proof of state-required auto insurance. Utilization of personal vehicle for travel within the community may be required. Approved mileage is paid at current IRS reimbursement rate.
Essential Functions
  • With support of the Outreach Manager, interview, hire and train street outreach professionals.
  • Provide direct supervision of outreach team members, including job orientation, coaching, performance evaluations, and support to implement best practice approaches and meet agency standards.
  • Partner with team members as needed to conduct professional street outreach within Indianapolis.
  • Complete client and program level documentation according to agency standards.
  • Complete Coordinated Entry System (CES) assessments as needed; represent team at CES case conferences to facilitate housing referrals according to Indianapolis Continuum of Care (CoC) guidelines.
  • Follow-up on referrals and coordinate with other outreach teams/providers to ensure continuity of care.
  • With support of Outreach Manager, identify and improve service delivery processes to maintain best practice case management.
  • Contribute to PIT Count planning process via Outreach Manager; participate in PIT Count.
  • Establish and maintain a positive and productive working relationship with community partners.
General Responsibilities
  • Support outreach team members as they serve individuals experiencing homelessness through direct client contacts, advocacy and empowerment, and collaboration with partners.
  • Maintain accurate electronic records and adhere to client confidentiality policies and procedures; and, utilize the Homeless Management Information System (HMIS) to record outcomes necessary for program functioning and evaluation.
  • Assist in program evaluation and internal compliance monitoring.
  • Participate in and contribute to all assigned internal and community-based meetings and trainings.
  • Accept responsibility for professional competency as appropriate to their discipline and agency standards.
