The Registered Nurse (RN) Case Manager will provide comprehensive care coordination and support to hospice patients and their families. The RN Case Manager will assess patient needs, develop individualized care plans, coordinate care services, and provide emotional support throughout the end‑of‑life journey.

• Clinical Assessment and Care Planning
• Symptom Management and Pain Control
• Emotional Support and Counseling
• Care Coordination and Collaboration
• Documentation and Compliance
• On‑call rotation for after‑hours and weekends

• RN licensure for the state of Indiana
• Clean driving record, valid driver’s license and auto insurance
