LPN Home Health Nurse – Travel Contract in Indianapolis, IN. Seeking a Licensed Practical Nurse (LPN) for a travel home health nursing job in Indianapolis, IN. This travel contract offers guaranteed hours, a supportive healthcare team, and the opportunity to deliver compassionate care to patients in their homes throughout Southwest Indianapolis, including Plainfield, Speedway, and Greenfield.

Compensation details are provided below.

• Deliver compassionate nursing care to patients in their homes across Southwest Indianapolis
• Complete accurate and timely clinical documentation using OASIS and HCHB
• Collaborate with interdisciplinary healthcare team to develop and implement individualized care plans
• Monitor patient progress and promptly report changes to supervising clinicians
• Educate patients and families on health management, preventive care, and chronic disease management

• Active Licensed Practical Nurse (LPN) license in Indiana
• At least 1 year of recent home health nursing experience (within the past 3 years)
• Proficiency with OASIS and HCHB (Homecare Homebase) documentation systems
• Strong communication and organizational skills
• Ability to meet productivity standards (minimum 30 points per week)
