Certified Nursing Assistant (Night‑Shift)
Creekside Health and Rehabilitation invites qualified candidates to join our compassionate team of licensed nursing assistants. This role focuses on delivering personalized, caring support to residents under the supervision and direction of a staff nurse.

- Receive and follow written or verbal care assignments, providing daily care for residents, including:
- Assisting residents with bathing, toileting, oral hygiene, skin care, grooming, nail care, and dressing needs
- Feeding residents, providing bedside water, snacks, and transporting them to dining areas in compliance with their nutritional plan
- Checking catheters frequently and providing catheter care as assigned by the nurse
- Assisting bedfast or comatose residents in turning, positioning, exercising, transferring, and ambulating per protocol
- Answering call lights and responding to requests for assistance in a timely manner according to facility policy
- Maintaining resident rooms by making beds, changing linens, cleaning bedside tables, and organizing personal belongings
- Observing and reporting changes in appetite, skin condition, or behaviors that deviate from the usual, per policy
- Offering emotional support and listening to residents to build trust and encourage independence
- Maintaining confidentiality of resident information to protect privacy rights
- Reporting and documenting incidents, accidents, complaints, or allegations with the occurrence reporting system
- Provide medical record documentation, including:
- Taking and monitoring vital signs, intake and outputs, weights, and meal consumption as assigned by the staff nurse
- Complying with documentation requirements for functional maintenance plans, 24‑hour reports, CNA flow sheets, intake logs, behavior management records, and other assigned records
- Comply with safety, infection control, and cleanliness standards, such as:
- Using protective devices (side rails, Geri chairs, Posey wheels) appropriately under nurse direction
- Abiding by emergency protocols for crash carts, codes, fire safety, resident codes, tornado preparation, violent residents, wander guard alarms, door alarms, and facility policies
- Reporting abuse witnessed or suspected to the nurse manager immediately
- Following infection control and universal precautions procedures
- Adhering to cleaning schedules for equipment and returning equipment to proper areas
- Maintaining a clean and sanitary nursing service work area
- Perform other duties as assigned by staff nurse or designee
- High school diploma or GED, unless working under a student permit
- Certified by the Indiana State Department of Health as a C.N.A. in good standing (including those transferring from other states or awaiting Indiana certification)
- Basic computer skills and familiarity with programs like Excel and Word
- Understanding of safe and effective nursing practices in Indiana
- Interpersonal skills:
- Assisting licensed nurse with procedures within scope
- Following written and verbal instructions, functioning independently, and making decisions when warranted
- Personal integrity and tactful interaction with residents, families, visitors, agencies, and personnel
- Harmony with professional and non‑professional personnel
- Judgment in prioritizing tasks, duties, and responsibilities per policies and procedures
- Discrete confidentiality, HIPAA, and corporate compliance adherence
- Patience, cheerful disposition, enthusiasm, and willingness to adapt to residents’ maturity and physical needs
- Adherence to professional ethics and ability to relay resident condition information to staff
