Parker Health and Rehabilitation Center, an 80-bed skilled nursing home, is looking for Licensed Practical Nurses to provide compassionate care to residents.

• Provide daily care, health needs, safety and comfort for residents.
• Conduct professional assessments and documentation of resident health.
• Assist with the management of assigned nursing unit staff.
• Participate in development and implementation of resident care plans; maintain up-to-date nursing care plans and make recommendations for revisions.
• Observe residents for symptoms, medication reactions, and document vital signs, intake/output, weight, diet supplements, cultures, etc.
• Assist in orientation and ongoing training of staff; ensure required skill levels are maintained.
• Report and document resident/visitor incidents, accidents, complaints or allegations via the designated occurrence reporting system.

• Graduated from an accredited nursing school with a valid LPN license in good standing (State and Federal requirements).
• Thorough understanding of safe, effective nursing practices within the State of Indiana.
• Basic computer skills, including Excel, Word, etc.
• Personal integrity and ability to deal tactfully with residents, family members, visitors, government agencies, and personnel.
