Personal Care Assistant

JOB DESCRIPTION & QUALIFICATIONS
Flexible Schedule. Bi-weekly Direct Deposit.

To provide excellent, one-on-one in-home patient care. We are looking to hire an honest, caring, and compassionate individual for this position.

• Assists, performs, and educates the patient/client and their family in activities of daily living, such as nail and skincare, grooming, oral hygiene, toileting and incontinence, and proper nutritional intake
• Assists with meal planning and preparation, shopping, social visits and activities, and maintaining a clean and safe physical environment for the client
• Assists in medication reminders and administration for drugs ordinarily self-administered
• Performs and completes assigned tasks according to the assignment sheet/service plan in a timely and professional manner
• Identifies and records changes in the patient/client’s condition and safety to the supervisor
• Completes mandatory Gentle mind Home Care in-services and provides requested documents to keep employee files current.

• Must be eighteen (18) years of age or older
• CPR certification will be required
• Can communicate effectively, which includes the ability to read, write, and verbally report clinical information to clients, their caregivers and family, and the agency staff
• Can perform and prioritize multiple tasks or functions
• Can provide proof of passing a written knowledge/competency exam
• Can provide proof of valid driver’s license and valid auto insurance
• At least one year experience in a similar job classification preferred
• Can constantly travel within geographic area serviced by the office from assignment to assignment
• Work is performed in the client’s home
• Can constantly stand, bend, stoop, twist, stretch, squat, kneel, walk, climb stairs, and reach freely
• Can frequently lift, reposition, and transfer clients and assist with standing and walking
• Can frequently carry loads, like groceries and laundry, up or down the stairs with or without assistance
