Position Overview:
IU Health is seeking an RN Care Manager to join our Kidney Care team. This position primarily follows a Monday to Friday, day shift schedule. The role will collaborate with another Care Manager to serve Nephrology offices at IU North, West, Fishers, University and Methodist. This is an onsite position, embedded in the practices.

As a Care Manager, you will provide exceptional care management services to Kidney Care patient populations, working collaboratively with a multidisciplinary team. You will serve as an advocate, guide, and resource for patients, families, and caregivers, engaging them in appropriate care management programs and implementing patient-centered interventions. Your goal will be to positively impact overall health outcomes as measured by clinical quality and utilization metrics, while contributing to broader population health objectives through continuous improvement methodologies.
Qualifications- Associate s Degree in Nursing required. New ASN RNs hired after January 1, 2013, will be required to complete their BSN within 5 years of hire date.
- Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
- Certification in Care Management preferred.
- Strong professional level knowledge of comprehensive clinical assessment skills preferred.
- Knowledge of Medicare/Medicaid, insurance and regulatory guidelines is preferred.
- 3-5 years of relevant experience required.
- Kidney Care experience is strongly preferred.
- Previous Utilization Management, Population Health, or Case Management experience is preferred.
