Licensed Practical Nurse – Rehab Care Center – Part Time

Under the direction and supervision of an RN/GN, the LPN/GPN will utilize practical nursing knowledge and address age‑specific needs as part of the care and treatment of Rehab patients. The LPN/GPN will be an active member of the interdisciplinary treatment team, collaborating in the development, implementation and revision of each patient’s interdisciplinary treatment plan. They will observe and evaluate patients, document observations and evaluations, and provide the RN with relevant clinical information.
Responsibilities- Provide direct patient care to Rehab patients.
- Collaborate with RN/GN and interdisciplinary team on treatment plans.
- Observe, evaluate, and document patient status.
- Deliver patient and family education.
- Maintain accurate clinical records and relevant documentation.
- Graduate of an accredited LPN program.
- Minimum 1‑year medical‑surgical experience preferred.
- Current LPN license in the Commonwealth of Pennsylvania.
- BLS certification (or obtain within one month of hire).
- Strong nursing knowledge in medical, surgical, and behavioral health.
- Excellent communication, writing, computer, and interpersonal skills.
- Ability to work collaboratively within an interdisciplinary team.
- Education:
LPN program graduate. - Experience:
1 year medical‑surgical (preferred). - Licensure: LPN, Pennsylvania.
- Certification: BLS.
- Shift: Rotation/mainly nights.
- Employment type:
