Salary Range Starting at: $20.20/ per hour
Job Objective:
Provides case management and care coordination services focused on enabling residents to live independently and assume full responsibility and control of their lives; provides counseling services to assist residents, including adults and children, with adjustment to residential living; through individual and group sessions, assesses residents' needs (including adults and children) for additional services. Services include street outreach and continuing support services.
What You Will Do:
Develops a case plan for each resident within 2 business days of admission to Center; case plan to include planning, including but not limited to the following areas:
- Housing
- Employment and job readiness
- Legal services
- Personal finance
- Public assistance
- Health, including mental health
- Childcare
- Transportation
- Material and financial assistance
- Enhancement of family functioning
- Outreach services
- Continuing support services
Assists residents in implementing the case plan by regular (at least 3 to 4 times per week) individual counseling sessions focused on:
- Providing information concerning community resources
- Encouraging and following up with residents to implement the details of the case plan
- Encouraging and assisting clients in following rules in the Center Handbook
- Actively functions as a mediator in situations of conflict
Functions as a liaison on behalf of residents with other agencies; serves as an advocate for residents and clients of the Center as needed. Including but not limited to IPATH, OUTREACH, DVN
May be asked to lead educational, instructional, or support groups in one or more of the following areas within the Center
- Domestic violence support
- Women in Recovery
- Children's' groups
- Community Resources group
Facilitates residents' adjustment to Center living by:
- Initiating and maintaining communication with all Center staff on issues relating to the adjustment of residents to the Center
- Ensuring adequate communication of the Center's expectations of residents; reads the Handbook to residents if needed
- Function in partnership with staff to enhance positive behavior and adjustment of children to the Center
- Function in partnership with Advocates to enhance and ensure stability and safety to consumers in the shelter
May be asked to conduct educational, instructional, and support groups in one or more of the following areas in the community:
- Domestic violence awareness
- Prevention of homelessness
Offers Life Skills training for all residents for an hour each day
- Plans, researches, and teaches topical information
- Coordinates outside resources (speakers, videos, etc).
Conducts street outreach to engage individuals experiencing homelessness and connect them with shelter, safety planning, and supportive services.
- Conduct proactive street outreach to individuals and families experiencing homelessness in encampments, public spaces, and other unsheltered locations.
- Build rapport and trust with clients through trauma-informed, person-centered engagement.
- Provide crisis intervention, safety planning, and immediate supportive services to individuals at risk of harm, including victims of domestic violence.
- Assess client needs, including shelter, housing, mental health, substance use treatment, medical care, employment, benefits access, and childcare.
- Connect clients with The Salvation Army WCC shelter and coordinate transportation or referrals when appropriate.
- Distribute essential items (food, hygiene kits, seasonal items, safety resources) to meet the urgent needs of unsheltered individuals.
- Maintain accurate and timely documentation of all outreach contacts, referrals, and services provided, including HMIS or other required systems.
