Key Responsibilities
* Provide community-based mental health services to individuals across the lifespan, including assessments, treatment planning, and intervention implementation.
* Collaborate with clients, families, and multidisciplinary teams to create and deliver individualized care plans that emphasize empowerment, resilience, and personal growth.
* Utilize evidence-based practices such as Motivational Interviewing, Trauma-Informed Care, Positive Reinforcement Techniques, and skills rooted in CBT and DBT frameworks.
* Offer behavioral interventions, including cognitive processing, group dynamics, and operant conditioning, tailored to client needs.
* Coordinate care with external providers such as primary care physicians, psychiatrists, educators, and local organizations to ensure a holistic approach to client support.
* Facilitate and lead group sessions related to psycho-social rehabilitation (PSR), based on client treatment goals.
* Maintain accurate and timely documentation of client interactions, progress, and treatment outcomes, adhering to organizational policies and legal requirements.
* Participate in professional development opportunities, staying current on best practices in mental health care and evidence-based techniques.
* Actively contribute to quality improvement initiatives through data collection, reporting, and collaborative efforts.
* Develop and maintain strong relationships with community stakeholders, including families, educators, and other key influencers in clients' support systems.
* Respond effectively to client crises, ensuring appropriate resources and interventions are in place.
Knowledge, Skills, and Abilities
* In-depth understanding of case management and rehabilitation principles for mental health and/or substance use disorders.
* Familiarity with community resources and the ability to connect clients to appropriate services.
* Knowledge of evidence-based practices, legal requirements, and court procedures related to mental health and substance use treatment.
* Strong interpersonal and communication skills, both verbal and written, with the ability to produce clear and concise documentation.
* Self-motivated with the ability to balance multiple priorities and maintain a high standard of service.
* Proven ability to establish effective relationships with clients, families, and professional teams.
For ACT/ACT TAY/IMPART teams:
Demonstrate understanding of the Assertive Community Treatment model, including its core processes, interdisciplinary collaboration, and service delivery.
Experience and/or Education
Qualifications:
* Bachelor's degree in Psychology, Social Work, or a related human services field; or
* Bachelor's degree in an unrelated field with two years of relevant experience; or
* Any combination of four years of higher education and related experience; or
* Associate of Applied Science in Behavioral Health…
