As a full-time Nurse Practitioner with Matrix Medical Network, you’ll conduct in-home and skilled nursing facility health assessments for adult and geriatric patients. Your work will directly improve health outcomes and close care gaps, all while enjoying the flexibility of travel across assigned states!

In this role, you will:
- Conduct thorough adult and geriatric assessments, including medical history, diagnosis, treatment, and health education.
- Administer and order necessary point-of-care and lab-based testing.
- Collaborate with physicians, case managers, and caregivers to ensure comprehensive patient care.
- Provide services in the comfort of members’ homes or Skilled Nursing Facilities.
Skills & Experience That Shine:
- Master’s Degree or equivalent experience with NP licensure.
- Current NP licensure with prescription authority or ability to obtain it.
- AANP, AACN, or ANCC board certification in relevant specialties.
- Current BLS, ACLS, or CPR certification.
- At least 1 year of NP experience.
- Able to travel 80-90% of the time, including overnight and weekend travel.
- Strong computer skills and excellent communication abilities.
- Consistency: Enjoy a typical Monday to Friday schedule from 8:00 AM to 5:30 PM.
- Competitive Pay: Earn an annual base salary ranging from $105,500 - $168,400 .
