Responsibilities

About the Role

During visits that can last up to one hour, you will:

  • Conduct adult and geriatric assessments, including medical history, diagnosis, treatment, health education, physician referrals, and case management.
  • Administer and order point-of-care and lab-based testing as needed.
  • Collaborate with physicians, case managers, social workers, family members, and other medical personnel.
  • Provide services at the member’s residence, whether in their homes or Skilled Nursing Facilities.

Qualifications

Skills & Experience That Shine

  • Master’s Degree or equivalent experience with NP licensure.
  • Current NP licensure in your state of practice, including prescription authority.
  • AANP, AACN, or ANCC board certification in relevant specialties.
  • Current BLS, ACLS, or CPR certification.
  • 1 year of NP experience.
  • Able to travel up to 80-90% of the time, including overnight and weekend travel.
  • Valid state driver's license and proof of adequate automobile insurance.
  • Strong computer skills and familiarity with health/medical record software.
  • Excellent verbal and written communication skills.
