Position Overview
The Health Care Navigator II connects veterans to VA health care benefits and community services. The role involves care coordination, health education, interdisciplinary collaboration, and administrative duties, ensuring timely, equitable, and veteran-centered care.
Key Responsibilities- Serve as liaison between U.S.VETS programs and VA/community medical clinics.
- Coordinate care for veterans with complex needs, including enrollment, documentation, appointments, and transportation.
- Conduct non-clinical assessments to identify barriers to care.
- Support veterans in expressing care preferences and health goals; educate and support families.
- Develop and maintain care plans with a focus on outreach and referrals.
- Act as a health coach to optimize treatment outcomes; coordinate complementary health and wellness services; link veterans to supportive services such as housing, financial benefits, and transportation.
- Provide ongoing education and support to veterans and families to address health literacy needs.
- Collaborate and consult with interdisciplinary team members, maintaining professional boundaries and adhering to ethical standards (HIPAA, Duty to Warn, etc.).
- Enter data and case notes into HMIS and other platforms; assist in policy development and program evaluation; provide expert consultation to staff and community providers.
- Maintain knowledge of SSVF program eligibility, goals, and regulations; share resources and coordinate care across all U.S.VETS programs.
- Bachelor’s degree in social work or related field.
- 2–3 years of experience in healthcare or medical social work.
- Experience working with low-income or homeless populations.
- Proficiency in Microsoft Office 365 (Outlook, Word, Excel).
- Ability to work flexible hours, including evenings and weekends.
- Reliable transportation, valid driver’s license, and ability to meet insurance requirements.
- Master’s degree in social work or equivalent.
- Clinical Social Work license.
- Familiarity with Veteran’s Health Administration.
- Strong communication and listening skills.
- High integrity and ethical standards.
- Self-motivated, organized, and accountable.
- Ability to work across diverse settings and roles.
- Commitment to quality control and continuous improvement.

Location:
Los Angeles, CA | Salary: $65,000 - $68,000
