We are a mission-driven healthcare organization dedicated to improving health outcomes for vulnerable populations by addressing social determinants of health. We seek a visionary LCSW to design, launch, and oversee a comprehensive mental health and psychosocial support program serving transitional age youth (16-25), individuals experiencing homelessness, older adults (65+), and other underserved groups.
Position OverviewThis leadership role combines clinical social work expertise with program management to build initiative(s) from the ground up. You will supervise a multidisciplinary team, deliver trauma-informed services, establish community partnerships, and ensure program compliance and outcomes achievement.
Key Responsibilities- Design, launch, and oversee all program operations including policies, protocols, budgets, and strategic partnerships
- Recruit, hire, train, and supervise clinical and support staff; provide clinical supervision and professional development
- Ensure delivery of trauma-informed, culturally responsive mental health services including assessments, individual support, crisis intervention, and resource navigation
- Manage all data collection, outcome measurement (BUPPS, MOQA, satisfaction surveys), compliance, and funder reporting
- Track and achieve KPIs across engagement, service delivery, referrals/linkages, community outreach, and equity metrics
- Build community partnerships, host events, and deliver training on trauma-informed care and mental health awareness
- MSW from CSWE-accredited program and current LCSW license
- 5+ years post-licensure clinical experience with vulnerable populations (TAY, unhoused, older adults)
- 3+ years program management/leadership experience including staff supervision and program development
- Expertise in trauma-informed care, crisis intervention, and addressing social determinants of health
- Strong project management, data analysis, and reporting skills; proficiency with CRM systems and outcome measurement
- Experience with grant-funded programs, compliance, and funder requirements
- Deep commitment to social justice, health equity, and cultural humility
- Doctoral degree, trauma certification (EMDR, DBT, CBT), eCPR training, PMP certification, experience with populations served, community mental health or homeless services experience.
