RN Case Manager

AMN Healthcare is hiring an RN Case Manager. The role begins 1/12/2026 with a pay rate of $1,800.00 – $2,700.00.

• Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization
• Patient assessment
• Referrals to HHC, SNF, A‑Rehab, SNF, LTAC, LTC
• Arrange DME
• Arrange IV antibiotics if needed
• Arrange acute to acute transfers
• Arrange transportation on discharge
• Participate in daily MDR

• FL/compact RN license
• BLS certification
• 2 years recent acute hospital case management experience
• Cerner & Midas experience

• All Inter Qual courses completed and test passed before starting

Length of assignment: 13 weeks.

Shift: 8:00 A – 4:30 P
