Clinical Pharmacist – Acute Care ICU PM 70–100%

University of Iowa Health Care, Department of Pharmaceutical Care is seeking a Clinical Pharmacist for Critical Care. The full‑time pharmacist will provide comprehensive pharmaceutical care for adult patients in adult critical care units including Cardiovascular, Medical, Surgical, Neurosciences, and Burn ICUs.

The role is offered at 70%–100% effort (second‑shift weekdays 1.0 FTE or 7‑on‑7‑off 0.7 FTE) with holiday staffing rotation. Pharmacies can work up to three holidays annually, and compensatory days off are provided if a weekend is worked with holiday coverage.

Selective candidates may receive a sign‑on bonus of up to $10,000.

Position Responsibilities
  • Provide therapeutic drug monitoring.
  • Evaluate medication orders for accuracy and appropriateness.
  • Ensure pharmacokinetic assessment of drug dosing on appropriate patients.
  • Communicate medication needs to prescribers.
  • Identify and evaluate adverse drug reactions.
  • Provide drug information to healthcare professionals.
  • Confirm and clarify patient drug allergies noted in the medical record.
  • Complete medication histories and reconcile home medications with admission orders.
  • Review and reconcile all medications upon patient transfer between clinical services.
  • Review daily therapeutic monitoring information for all patients in the unit(s).
  • Identify high‑risk medication therapy per admission criteria and review as appropriate.
  • Complete required competency exams as required by the department.
Documents Patient Care Activities Performed
  • Document medication error information into standard reporting system.
  • Identify, evaluate, and take steps to avoid adverse drug reactions.
  • Update patient allergies and patient‑specific height/weight information when needed.
  • Ensure correct medication histories are entered into the medical record.
  • Provide and document medication counseling to patients for targeted populations.
  • Document therapeutic drug monitoring assessments and recommendations.
Coordinates Patient Communications/Interactions
  • Perform medication history and reconcile home medications with inpatient orders.
  • Discuss drug allergy history.
  • Conduct patient teaching classes (where appropriate).
  • Assist with discharge medication reconciliation and pending for targeted populations.
  • Provide medication counseling at discharge for high‑risk populations.
Spearheads Patient Care Initiatives
  • Coordinate medication orders written during assigned hours of duty.
  • Follow‑up on medication system problems.
  • Carry out thorough interventions and education, protocol antibiotic, and DUE initiatives.
  • Participate in multidisciplinary initiatives.
  • Serve on intra‑ and inter‑departmental committees as required.
  • Coordinate discharge planning activities.
Provides Educational Services

The Clinical Pharmacist is responsible for education of students, residents, fellows, staff, and patients regarding drug use. Educational activities include:

  • Preceptor for pharmacy students and residents.
  • Presenting in‑service educational programs to staff nurses, physicians, and other healthcare team colleagues.
  • Presenting at educational programs sponsored by the Department, College of Pharmacy, other UI Health Care departments, and state and national organizations.
  • Conducting educational classes for pharmacy students and residents.
  • Conducting patient education classes about proper medication use.
  • Serving as mentor and role model for other pharmacist practitioners within the department.
Education Requirements
  • Pharm

    D (Doctor of Pharmacy) degree or Bachelor of Science in Pharmacy.
Experience Requirements
  • One (1) year of applicable inpatient hospital pharmacy experience, including pharmacy residency.
  • Current licensure by the Iowa Board of Pharmacy Examiners or eligibility to obtain licensure.
  • Excellent written and verbal communication skills.
  • Experience related to and commitment to service excellence.
Desired Qualifications
  • Experience in hospital pharmacy setting.
  • BLS and ACLS certification or eligibility for completion.
  • Completion of an ASHP‑accredited PGY1 residency.
  • Completion of an ASHP‑accredited PGY2 residency in Critical Care or another specialty.
