Hourly Wage: New Hire: $27.13

About Social Worker: Provides in‑home support and resource coordination to families with youth under 18 to promote stability, improve adult mental health, and enhance youth success at home and in school. This role involves direct engagement with families, schools, and community partners to deliver early intervention, skill‑building, and access to critical services that support overall family well‑being and child development.

  • Conducts weekly in‑person visits with families in their homes or other preferred locations.
  • Assists families in accessing and navigating community resources including SNAP, Medicaid, employment, and mental health supports.
  • Coordinates transportation services or provides direct transport to appointments when necessary.
  • Supports clients in completing paperwork, applications, and phone calls required to access services.
  • Maintains regular contact to ensure follow‑through on appointments, medication management, and service plan goals.
  • Delivers weekly skill‑building sessions for youth and coaches families to improve communication with schools.
  • Attends IEP meetings and follows up with educators to support youth progress.
  • Maintains accurate and timely documentation in Evolv, Dayforce, and Excel.
  • Participates in agency meetings, trainings, and community outreach.
  • Meets regularly with supervisors to align services with program objectives.
  • Bachelor’s degree in Social Work or an approved related human service field degree required.
  • Minimum of two (2) years of full‑time experience or equivalent required serving children, adolescents and/or their families in their home.
  • Valid State of Iowa Driver's License required.

Monday – Friday, 8:30 a.m. – 5 p.m.
