Job Description

Occupational Therapist—An Occupational Therapist at Elara Caring will continue to provide high‑quality care to more than 60,000 patients each day in their preferred home setting. You will have a variety of ways to make a difference through developing and delivering expert care.

Key Responsibilities
  • Evaluate occupational therapy patients to determine rehabilitation needs, potential for achievement, and develop a plan of care.
  • Participate in the implementation and development of the Plan of Care to ensure quality and continuity of care and proper discharge planning.
  • Conduct initial therapy evaluations and re‑evaluate patients’ therapy needs during each visit, making follow‑up assessments according to OASIS and PPS regulatory guidelines.
  • Communicate significant findings, problems, changes in condition or environment to the appropriate supervisor, physician, and other team members involved with patient care.
  • Initiate a plan for patient safety using patient, family, and community resources.
  • Verify the Plan of Care prior to each visit and provide care according to physician’s orders, assessment data, and established standards and guidelines.
  • Write physician orders to cover additional visits and changes to the Plan of Care, per agency policy.
  • Incorporate patient care goals established in the Plan of Care into therapy care, as evidenced by documentation in therapy notes.
  • Perform occupational therapy services in accordance with accepted standards of practice and certified by the patient’s physician.
  • Instruct patients and/or their families with activities of daily living, utilizing adaptive equipment and sensory integration and fine‑motor coordination skills.
  • Assist patients and caregivers in securing necessary equipment, including fabrication of custom‑made splints.
  • Provide patient and caregiver instruction in the therapy home program, offering modification and clarification when necessary.
  • Counsel and instruct patients and families in meeting therapy and related needs.
  • Supervise and/or provide clinical direction to the Certified Occupational Therapy Assistant (COTA), Home Health Aide (HHA), and clinical staff to ensure the therapy care plan is followed and quality of care is maintained.
  • Physically demanding, high‑stress environment.
  • Full range of body motion including handling, lifting, and transferring patients.
  • Potentially work irregular hours, including call hours if applicable.
Requirements
  • Graduate of an accredited Occupational Therapy program.
  • Current, unrestricted OT license valid for the state of work.
  • Minimum one (1) year of experience in a clinical care setting.
  • Able and willing to travel within the branch/office coverage area.
  • Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
  • Must be able and willing to travel 50% of the time.
  • Able to sit, stand, bend, lift, and move intermittently and be able to lift 50–100 lbs.

You will report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee.
