Eagle Ridge Health & Wellness and Foxglove Wellness & Rehabilitation are independently owned Skilled Nursing Facilities entering a new chapter focused on clinical excellence, compassion, and resident-centered rehabilitation. We are seeking a dedicated and motivated Occupational Therapist (OT) to join our therapy team and help residents regain independence, confidence, and functional ability in their daily lives.

• Evaluate residents to determine functional abilities, rehabilitation needs, and individualized treatment goals

• Develop and implement evidence-based occupational therapy plans of care

• Provide therapeutic interventions focused on ADLs, fine motor skills, cognitive functioning, adaptive equipment use, and safe engagement in daily routines

• Train residents in compensatory strategies to maximize independence

• Educate residents, families, and staff on safe techniques, adaptive equipment, and functional support strategies

• Document evaluations, treatments, progress notes, and discharge summaries accurately and timely

• Collaborate with nursing, physical therapy, speech therapy, and administrative teams to ensure coordinated, high-quality care

• Participate in care plan meetings and contribute to interdisciplinary discussions

• Maintain compliance with all regulatory, clinical, and safety standards

• Current Occupational Therapist (OT) license in the state of Iowa

• Strong clinical reasoning and assessment skills
