Job SummaryThe Nursing Assistant (NA) serves as caregiver to the resident during the scheduled work period in long term care. Provides resident-centered nursing care and daily living assistance to assigned resident under the supervision of a registered nurse (RN). Delivers age-appropriate care related to the physical and psychological needs of the resident as per care plan. Maintains infection prevention and control policies and uses Personal Protective Equipment as required.
Provides assistance with basic health care needs including bathing, toileting, grooming, dressing/undressing, obtaining and recording vital signs, and providing psychosocial support and other personal care. Assists residents with transfers, repositioning, and walking using correct techniques and equipment. Provides range of motion and passive exercises. Supports dining activities and assists with meals and snacks as appropriate. Documents resident interactions and communicates changes in condition to the charge nurse.
May collect non-invasive specimens and assist with wellness and recreational programs. May participate in committees or task forces as needed.
- Provide resident-centered nursing care and daily living assistance under RN supervision
- Assist with bathing, grooming, dressing, toileting, and feeding
- Record vital signs and assist with sample collection as needed
- Assist with transferring, positioning, mobility, and ROM exercises
- Maintain infection control practices and PPE usage
- Document care and communicate changes to nursing staff
- Support wellness and recreational activities
- High school graduate or GED preferred, but not required
- Minimum age 16
- Prior clinical or nursing assistant experience in long-term care preferred
- Required CNA certification with the State Board of Nursing, or enrollment/certification processing within four months
- Yearly competency testing and continuing education (minimum 12 hours annually)
- State-specific requirements (e.g., UAP registry where applicable)
- May be classified as a Sanford Category II Driver depending on role; valid driver’s license and medical qualification may be required
