Sage Group Health Care Staffing is seeking RN Home Health for travel/contract position in Tiffin, OH. Accepting both local and traveler candidates.

Pay Rate: $2080 Weekly Gross pay based on a 40-hr work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments
Unit: Home Health
Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri;
No weekend or call
Position Requirements:
- Registered Nurse: 2years
- Required - Recent/Current Home Health Nursing experience: 1 year within the last 3 years – Required
- OASIS experience: 1 year– Required
- Home Care Home Base(HCHB) charting experience – Required
- Must have valid
Driver’s License and Personal Auto Insurance
Education, License and Certification Requirements:
- ASN or an equivalent– Required
- BSN or an equivalent– Highly preferred
- RN License for Stateof Ohio or Compact
- Required - BLS Certification from AHA or American Red Cross
- Required
Job Description:
- Provide and coordinate care for assigned patients in comfort of their homes.
- Develop and implement nursing care plans including Start of Care and admissions as assigned.
- Support patient recovery and ability to stay at home - enabling them to spend quality time with their loved ones and doing the activities they enjoy.
- Empower patients and families to reach healthcare goals by educating them on disease management.
- Provide Home health nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
- Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
- Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
- Notify physicians of significant changes in status.
- Must communicate with the office in timely manner,
- Must document and complete charting for all patient visits in a timely manner in accordance with client policies and procedures.

All of Seneca County, Portions of Hancock, Huron, Erie, Wood, Sandusky, Wyandot, Crawford, and Richland
