Care Initiatives Hospice is seeking a compassionate Hospice Admissions Nurse to join our team and facilitate patient transitions of care during admission. The role involves meeting patients and families where they are most comfortable — at home, in long-term care, assisted living, or hospital — and coordinating with Central Intake, Clinical Manager, Program Director, and Community Representative.
