Position Overview The Licensed Practical Nurse (LPN) provides direct nursing care to residents in accordance with individualized plans of care, physician orders, applicable nursing standards, and facility policies. Under the direction and supervision of an RN as applicable, the LPN monitors resident health, administers medications and treatments, documents care, and communicates changes in condition to the appropriate members of the healthcare team.

The LPN also collaborates with nursing assistants and other caregivers to support safe, timely, resident-centered care. This position is an excellent opportunity for a nurse who values teamwork, takes pride in helping others, and wants to continue growing professionally.
