The LTSS Service Coordinator operates primarily in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training. Candidates must reside within a reasonable commuting distance from the posting location unless an accommodation is granted as required by law.
Responsibilities- Conduct face‑to‑face program assessments using pre‑defined tools and motivational interviewing techniques for waiver and behavioral/physical health needs.
- Identify members with potential clinical health care needs and coordinate case management with interdisciplinary teams.
- Manage non‑clinical needs for members with chronic illnesses, comorbidities, or disabilities to ensure cost‑effective utilization of long‑term services and supports.
- Document short and long‑term service and support goals in collaboration with the member’s chosen care team.
- Identify members who would benefit from alternative levels of service or other waiver programs.
- Serve as mentor or subject‑matter expert for new staff and assist in formal training and process improvement initiatives.
- Submit utilization/authorization requests with supporting documentation aligned with the care plan.
- Report critical incidents to internal and external parties, including Adult Protective Services and Law Enforcement.
- Assist with member grievances, appeals, fair hearings, and state audits.
- Travel to work sites and other locations as necessary.
- Bachelor’s degree and a minimum of 2 years of experience with a social work agency, or an equivalent combination of education and experience.
- BA/BS degree in a health‑care related field.
