Field:
This field-based role enables associates to primarily operate in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training. Candidates not within a reasonable commuting distance will not be considered for employment unless an accommodation is granted.
- Perform face‑to‑face program assessments, using predetermined tools and questions, applying motivational interviewing techniques to evaluate, coordinate and manage an individual's waiver and other needs.
- Use tools and identification processes to surface members with potential clinical health care needs, coordinating their care with the interdisciplinary team.
- Manage non‑clinical needs of members with chronic illnesses or disabilities to ensure efficient use of services.
- Document member’s short and long‑term service and support goals in collaboration with the member’s chosen care team.
- Identify members who could benefit from alternative service levels or other waiver programs.
- Serve as mentor, subject‑matter expert, or preceptor for new staff, and participate in process‑improvement initiatives.
- Submit utilization/authorization requests with supporting documentation.
- Report critical incidents to appropriate internal and external parties (state and county agencies).
- Assist in and participate in appeals, grievances, audits, and other member Hearings.
- Travel to worksite and other locations as necessary.
- BA/BS degree and a minimum of 2 years of experience working with a social work agency, or an equivalent combination of education and experience.
- Specific education, years, and type of experience may be required based on state law and contract requirements.
- BA/BS degree in a health‑care related field.
