Emergency​/Advanced Care Practitioner

Overview

We are looking to recruit an experienced minor injuries qualified Practitioner, passionate about patient‑centred care, to enhance primary care clinical skills. As an Emergency Care Practitioner at The Birches Medical Centre you will be responsible for on‑day assessment of both health and clinical needs of patients, using clinical judgement to conduct consultations from diagnosis to treatment, including referrals when required.

• Carry out daily consultations with patients or their representatives.
• Facilitate comprehensive patient history and, if needed, physical examinations.
• Identify potential diagnoses using expert knowledge and clinical judgement.
• Decide on and carry out treatment, referring to secondary care within local agreed pathways and protocols as appropriate.
• Prescribe medication treatment regimes in accordance with local formularies and practice prescribing protocols, and review guidelines when necessary.
• Independently assess, diagnose and manage patients with acute and chronic health conditions.
• Develop and implement evidence‑based management plans.
• Prescribe safely and effectively within your scope of practice.
• Deliver health promotion, preventative care and patient education.
• Manage same‑day urgent presentations as well as routine clinical appointments.
• Work collaboratively with the wider multidisciplinary team to deliver outstanding patient care.
• Contribute to clinical governance, quality improvement projects and service development initiatives.
• Support, mentor and share knowledge with colleagues where appropriate.

• Qualified Advanced Clinical Practitioner (ACP) or Advanced Nurse Practitioner (ANP).
• Current registration with the appropriate professional body.
• Excellent clinical assessment, diagnostic and decision‑making skills.
• Strong communication, interpersonal and team working abilities.
• Commitment to delivering safe, effective and compassionate patient care.
• Independent Prescribing qualification desirable.
• Experience in Primary Care highly advantageous.
• Registered Nurse or Paramedic, First Level Nurse or other healthcare professional qualification leading to registration.
• Degree level study or equivalent (evidence required).
• Ability to work autonomously.
• Independent non‑medical prescriber qualifications (evidence required).

• Past and present professional development, with examples where necessary.
• Experience working within Primary Care or GP Practice.
• Specialism in any long‑term condition management desirable but not essential.
• Other related experience, areas of expertise and skill sets also desirable but not essential.

Location:

Kesgrave, on the outskirts of Ipswich. The practice uses the Systm One clinical system with a list size of approximately 6,500 patients and is part of the new Wolsey PCN.
