• Support and educate ward staff and case managers to coordinate complex cases throughout the discharge planning process, avoiding unnecessary hospital stays and ensuring safe and timely discharge from hospital over a seven‑day service.
• Complete appropriate assessments (TNA, MCA, CCG referrals) in accordance with the DOH Framework for NHS Continuing Healthcare and DOH Hospital Discharge Policy, and provide signature for completion of Assessment of Nursing/Healthcare Needs.
• Act as support for junior staff and take a leading role in ward staff and team’s development regarding discharge process/pathways.

• Registered Nurse First Level
• Completion of mental health capacity training to facilitate mental health capacity assessments

• Proven experience as a Band 5 RGN/RMN
• Recent practice in hospital setting
• Supervising other staff
• Evidence of delivering appropriate standard of patient care and improving the patient experience
• Working within a multi‑disciplinary team/joint assessment team environment

• Understanding of the NMC code of professional conduct
• Knowledge of DOH Hospital Discharge Policy including discharge pathways and criteria to reside
• Excellent interpersonal skills
• Knowledge of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care
• Nurse signatory for Interim Nursing Care payment

£39,959 to £48,117 per annum pro rata
