Your role and responsibilities  Support the Home First team with customer assessments and reviews.

Work with allocated customers to identify and deliver high-quality, person-centred outcomes.

Build strong relationships with customers to achieve personalised goals and improve independence.

Work collaboratively with social work teams, health partners and the voluntary sector to support integrated care.

Make a meaningful difference to the lives of residents across Suffolk.

Receive regular support and professional supervision as part of a collaborative team environment.

You will need strong communication skills when working with customers, their families or advocates, and professional partners the ability and energy to respond to urgent work as part of a team system resilience under pressure and the ability to remain positive when facing challenges.
