Role Summary

As a House Lead (Key Worker), you will play a vital role in coordinating the day‑to‑day care and support of individuals within our supported living or residential services. Working alongside the Team Leader, you will ensure each person receives high‑quality, person‑centred support that promotes independence, choice, dignity, and wellbeing.

You will act as the primary Key Worker for allocated individuals, developing positive relationships with the people we support, their families, healthcare professionals and colleagues to ensure care is delivered consistently and effectively. You will also be responsible for maintaining accurate care records, supporting individual outcomes, and ensuring all care is delivered in line with Care Quality Commission (CQC) standards, legislation, and organisational policies.

In addition to your Key Worker responsibilities, you will continue to undertake all duties associated with our Support Worker role, providing hands‑on care and leading by example to deliver outstanding support.

Key Responsibilities Client Care & Support
  • Act as the Key Worker for allocated clients, taking responsibility for their care, wellbeing and Health Action Plans, ensuring these remain accurate, up to date and actioned.
  • Coordinate and attend health appointments, annual reviews, and multidisciplinary meetings as required.
  • Develop, implement, and review person‑centred care plans and risk assessments in partnership with clients, families, and professionals.
  • Promote independence, choice, dignity, and positive outcomes through high‑quality, person‑centred support.
  • Monitor clients' health, behaviour, and changing needs, adapting support where required.
  • Facilitate and record client reviews and meaningful engagement activities.
Documentation & Compliance
  • Maintain accurate, timely, and comprehensive care records, including daily notes, care plans, risk assessments, and BIRDIE documentation.
  • Ensure all documentation complies with CQC standards, legislation, and organisational policies.
  • Report safeguarding concerns promptly and follow all reporting procedures.
  • Complete and submit shadow shift documentation.
  • Support internal and external audits by maintaining accurate client records.
Staff Support & Teamwork
  • Work collaboratively with colleagues to deliver consistent, high‑quality care.
  • Support the Team Leader with staff inductions, shadow shifts, training, and staff allocation as required.
