- Work with families, schools and professionals to gather information for Education, Health and Care Needs Assessment requests
- Collate and organise information from multiple sources for presentation to a multi-disciplinary panel
- Build positive relationships with parents, carers, educational settings, health colleagues and other stakeholders
- Act as a key contact, respond to enquiries and provide updates throughout the assessment request process
- Maintain accurate records using data recording and case management systems
- Monitor and track requests to ensure processes progress within required timescales
- Collaborate with colleagues across the SEND service to support effective decision-making and quality outcomes
- Identify opportunities to improve processes and the experience of families and professionals
- Strong organisational skills and ability to manage and prioritise a varied workload
- Excellent communication skills and ability to build positive relationships with families, schools, professionals and colleagues
- Ability to gather, collate and present information clearly and accurately
- Keen eye for detail and commitment to maintaining accurate records across multiple systems
- Confidence using IT systems and databases, with ability to learn new systems and processes
- Collaborative approach and ability to work as part of a supportive team
- Ability to handle sensitive information with professionalism, discretion and empathy
- Good problem-solving skills
- Commitment to delivering high-quality services and positive outcomes for children, young people and families
- Understanding of the needs of children and young people with SEND and their families
- Supporting statement required; documents must be submitted in PDF or MS Word format
- DBS checks or police vetting required for relevant posts
- Commitment to safeguarding and promoting the welfare of children, young people and vulnerable adults
Demonstrates strong organizational and communication skills to effectively manage Education, Health and Care Needs Assessment requests while building positive relationships with families and professionals. Proficient in maintaining accurate records and collaborating with multi-disciplinary teams to ensure quality outcomes for children and young people with SEND.
